The examples below are de-identified illustrations, drawn from the kinds of gaps our field studies routinely surface. Specific commissioned reports are confidential to the publishers who ordered them; what follows is offered only to show the character of the work. Names of titles, publishers, and practitioners are withheld.


Illustration 1 · Allied Health

The textbook said: A patient hand-off followed a verbal checklist recited at shift change.

Practitioners reported: The unit had moved to a written, structured hand-off tool years earlier after the verbal method was tied to errors. Graduates trained on the old method were arriving unfamiliar with the form every floor now required them to complete.

Mark returned: Outdated.


Illustration 2 · Skilled Trades

The textbook said: A measurement was taken with a particular analog instrument, described over two full pages.

Practitioners reported: The analog instrument was still taught in apprenticeship but had been replaced on the job by a digital tool that logged readings automatically. Knowing the old instrument was useful; not knowing the new one was a liability on a first day.

Mark returned: Drifting. Retain the principle, add the current tool.


Illustration 3 · Business Practice

The textbook said: A reconciliation procedure was performed by hand across several ledgers.

Practitioners reported: The manual steps remained conceptually correct and were still worth teaching for understanding, but no working professional performed them by hand. The chapter's worked example, however, was accurate in principle and survived unchanged.

Mark returned: Current in concept, Drifting in practice. Flagged for the next edition.


Illustration 4 · Laboratory Technique

The textbook said: A safety step referenced a material that had since been restricted by regulation.

Practitioners reported: No working lab had used the material in years; a substitute and a changed procedure were now standard. This was the rare case where an outdated passage was not merely unhelpful but pointed a student toward a practice now prohibited.

Mark returned: Outdated. Recommended for revision before printing.


What these have in common

In none of these cases was the underlying theory wrong. In every case the surrounding practice had moved, and only someone doing the work could have said so. That is the entire premise of our firm.
